If you've been told you have tennis elbow — clinically, lateral epicondylitis or lateral elbow tendinopathy — and your job involves your hands, you've probably had a version of this thought:
I can't take months off. How am I supposed to recover if I can't stop using my arm?
It's one of the most common and most valid frustrations I hear. And the answer most people get — "rest it as much as possible" — is both impractical and, in most cases, wrong.
Here's what I actually tell patients who need to keep working through tennis elbow recovery.
The Rest Myth
Tennis elbow has a reputation as something that "just needs rest." This is partly because it can improve with rest — temporarily. But tendons don't recover by being left alone. They recover by being progressively loaded in a way they can tolerate and adapt to.
Complete rest weakens the tendon, reduces its load capacity, and often means the pain comes roaring back the moment you return to full activity. I've seen patients rest for three months and go right back to square one on their first week back at work.
The goal isn't to stop using your arm. It's to manage how you use it while building the tendon's capacity to handle that use.
Who's Most at Risk at Work?
Tennis elbow is a misleading name — the majority of people I see with it have never picked up a racket. The common thread is repetitive forearm and wrist loading. That includes:
Tradespeople (carpenters, plumbers, electricians), chefs and kitchen staff, healthcare workers (dental hygienists, massage therapists, surgeons), office workers with high keyboard and mouse use, hairdressers, and assembly line or factory workers.
If your job involves sustained gripping, repeated wrist extension, or forceful forearm rotation — that's the load profile that irritates the extensor tendons at the lateral elbow.
Modify, Don't Eliminate
The most realistic approach to working with tennis elbow is modification, not cessation. Here's what that looks like in practice:
Reduce force and repetition where you can. Can you take an extra tool to avoid one forceful movement? Can you alternate hands for lighter tasks? Small reductions in load add up over an eight-hour day.
Adjust your grip. A wide, tense grip dramatically increases forearm extensor activation. Try using a looser grip where possible, or wider-handled tools that reduce the gripping effort required.
Take micro-breaks. Brief, regular breaks (every 45–60 minutes) to shake out the arm, do a few gentle wrist movements, and reduce sustained loading are more effective than one long break mid-shift. The tendon needs intermittent recovery within the workday.
Ice at the end of the day. 10–15 minutes of ice on the lateral elbow after work can help manage the inflammatory response and reduce the accumulated irritability from the day's load.
My patients who do best are the ones who treat the work day as a load management problem — not a battle of willpower.
What About a Brace?
Counterforce braces — the strap-style bands worn around the forearm, just below the elbow — are commonly used for tennis elbow. The evidence on them is mixed, but many people find them genuinely helpful for getting through a workday with less pain.
My perspective: they're a reasonable short-term tool if they allow you to work with less discomfort and without dramatically altering movement patterns. But they don't treat the problem — they manage symptoms while you build the tendon's capacity through proper loading exercises.
Don't use the brace as a substitute for rehab. Use it as a bridge that makes rehab possible.
The Rehab You Need Alongside Work
This is where most people fall short. They modify work, maybe use a brace, and wait for it to get better. But without a progressive loading program, the tendon never builds the capacity it needs.
The foundation of tennis elbow rehabilitation is progressive wrist and forearm extensor loading. That means exercises like:
Wrist extension with a light weight or resistance band, progressed over time. Eccentric wrist extension (lowering slowly). Heavy slow resistance exercises as the tendon becomes less irritable. Grip strengthening to build load tolerance from a different angle.
These exercises should feel like mild discomfort during the set — not sharp or worsening pain — and settle within 24 hours. If you're flaring significantly after your exercises, the load is too high and needs to be dialed back.
Managing Flares at Work
There will be hard days — a project that demands more than usual, a weekend where you overdid it, or just a week where symptoms spike without obvious reason. Here's how to handle them:
Reduce but don't eliminate load. A significant flare is a signal to dial back by 30–40%, not to stop entirely. Complete rest after a flare often resets the irritability cycle and makes the next return to activity more provocative, not less.
Don't push through pain above 5/10. Some discomfort is acceptable — sharp, escalating, or lingering pain is a clear signal to back off.
Check your recovery inputs: sleep, stress, and general workload all influence tendon irritability. A bad week at work plus poor sleep plus two extra hours of gripping activity can tip an otherwise manageable tendon over the edge.
When to Get Proper Assessment
If you've been managing tennis elbow for more than 6–8 weeks without meaningful improvement — or if it keeps flaring back with any increase in work demand — it's worth getting a structured assessment and a proper loading program.
I see a lot of patients who've been modifying and managing for 6–12 months but have never actually been given a targeted rehab program. The tendon has been surviving, not recovering. Those cases take longer to turn around, but they do turn around with the right approach.
Tennis elbow doesn't have to derail your work. It does require a smarter approach than just toughing it out — or waiting for it to go away on its own.
