Of all the patients I see with tennis elbow, most of them have never held a tennis racket in their lives.
There's a plumber who developed it from years of pipe wrenching. A project manager who got it from mouse use and typing. A hairdresser who's been cutting hair for fifteen years. A mum who's been carrying a toddler on one hip for the last two years.
Tennis elbow — the clinical term is lateral epicondylitis, or more accurately, lateral elbow tendinopathy — is one of the most common upper limb injuries in working-age adults. And the name is probably the most misleading label in all of musculoskeletal medicine.
What Tennis Elbow Actually Is
The pain sits on the outside of the elbow, usually right at or just below the bony bump (the lateral epicondyle). That's where several forearm extensor tendons attach — the muscles that extend your wrist and fingers.
When those tendons are repetitively loaded beyond their capacity to recover, the tendon structure starts to change. The research has moved away from the older "tendonitis" model (inflammation as the primary driver) and toward a degenerative or dysregulative model — which is why the more accurate term is tendinopathy rather than tendonitis. But patients search for "tennis elbow" and "elbow tendonitis," so those terms still matter for communication, even if they're not clinically precise.
The tendon doesn't know whether you're holding a racket, a wrench, or a mouse. It knows how much force you're putting through it and how much time it has to recover.
The Real Causes: It's About Load and Recovery
Tennis elbow doesn't appear out of nowhere. There's almost always a load story — either an increase in what you're doing, or a prolonged period of something your forearm wasn't conditioned for.
Common culprits I see:
Trades work. Plumbers, electricians, carpenters, mechanics — anyone using wrenches, screwdrivers, gripping tools for hours at a time. The forearm extensors are working constantly without adequate rest.
Computer use. A lot of people underestimate how much sustained forearm extensor activity happens during prolonged mouse use, especially with a stiff, over-gripped hand position. I've seen people develop clear lateral elbow tendinopathy from desk work alone.
Manual labour changes. A warehouse job, moving house, a renovation project — a sudden spike in gripping and lifting that the tendon wasn't prepared for.
Parenting. Picking up and carrying children repeatedly, particularly with the wrist in extension, is a surprisingly common cause in new parents.
Racket sports, gym work, or new hobbies. Yes, tennis is on the list — but so is weightlifting, golf, rowing, and anything that involves sustained or repetitive forearm loading.
Why Rest Doesn't Fix It
This is where most people get stuck. They rest, it feels better. They go back to work or activity, and within a few days it's back. They rest again. The cycle repeats for months — sometimes years.
Rest reduces load below the symptom threshold. But it doesn't rebuild the tendon's capacity. The tendon remains undertrained for what you're asking of it. The moment you return to normal activity, you're right back at the edge of its tolerance.
I see this pattern constantly. Someone who's had tennis elbow for a year or more has usually tried extended rest multiple times. The pain keeps coming back because the underlying problem — an undertrained tendon — hasn't been addressed.
What Actually Works: Progressive Tendon Loading
The evidence for tendinopathy management consistently points to progressive loading as the primary treatment. For lateral elbow tendinopathy, this means exercises targeting the wrist extensors — specifically, wrist extension exercises with progressive load.
A few things that matter:
Isometrics first. In the early stages, sustained isometric contractions (holding a position under load without movement) can reduce pain and start the process of tendon conditioning with lower provocative risk.
Eccentric and heavy slow resistance loading. As symptoms allow, progress to isotonic exercises — slow wrist extensions and flexions with weight. The load needs to be enough to challenge the tendon, not just go through the motions with a rubber band.
Grip strengthening. Because gripping is usually what provokes the problem, it seems counterintuitive — but gradually rebuilding grip capacity is essential for function. The tendon needs to be conditioned for the actual demands you're placing on it.
Load management at work or in daily life. Reducing provocative loads during the early stages of rehab isn't giving up — it's creating a window for the tendon to start adapting. This doesn't have to mean time off work. It often means modifying how you do certain tasks temporarily.
What Probably Won't Fix It
I want to be honest about some commonly sought interventions:
Cortisone injections. They provide short-term pain relief — sometimes significant relief. But the evidence on long-term outcomes is not favourable. Multiple studies have shown that cortisone outperforms physiotherapy and wait-and-see at six weeks, but at 12 and 52 weeks the physiotherapy group does better. And repeated cortisone injections may actually weaken tendon structure over time.
Elbow braces (epicondyle straps). Counterforce braces can help manage symptoms during activity by altering load distribution. They're a useful short-term tool. They don't address the underlying problem and shouldn't be relied on long-term.
Passive treatments alone. Massage, ultrasound, laser, and dry needling can support symptom management and are useful adjuncts. But if they're the whole programme — without progressive loading — outcomes tend to plateau.
How Long Does Recovery Take?
This is the honest part. Lateral elbow tendinopathy can be stubborn. Mild cases with good rehab compliance often resolve meaningfully in 8–12 weeks. More chronic cases — especially those with years of symptoms and multiple treatment failures — can take 4–6 months of consistent work.
I know that's not what anyone wants to hear. But setting realistic expectations is better than cycling through failed short-term treatments. The tendon needs time to remodel, and the process can't be dramatically rushed.
What I can tell you is that consistent, progressive loading — done correctly — is the most reliable path to genuine recovery. Not full relief in week two, but meaningful improvement over months, and a tendon that's actually conditioned to handle what you throw at it.
You didn't develop tennis elbow overnight. Rebuilding the tendon's capacity takes time — but the tendon does respond when you give it the right stimulus.
Where to Start
If you've been dealing with lateral elbow pain and the rest-and-wait approach isn't working, it's worth getting an assessment to rule out anything else (cervical referral, radial nerve involvement, and joint pathology can all mimic lateral elbow tendinopathy) and then starting a structured loading program.
The exercises themselves aren't complicated. The key is progression — gradually increasing load and demand over time in a way the tendon can adapt to. That requires some structure and monitoring, not just YouTube stretches.
